two strokes within two weeks

At the age of 82.
Now grandmother is in a "old age" house. She can barely walk by herself and only in the morning; diagnosed with dementia after the stroke(s). She also has diabetes and a heart surgery, one or two heart failures on her record...
I must say she used to be *very* active though, even after her heart attack and surgery.
I am thinking it might help if daily visiting and taking her outside for walks, but not sure... she has had no actual rehab in 3-4 months since her first stroke. Is it possible in this situation to get here walking from here to there around the house ?
